Outside Astral Observatory - Talk to the Akindo Nut in Clock Town, then look through the telescope and witness him enter a Hidden Hole near the Observatory. Purchase for 100 or 150 Rupees.
Hidden Hole outside Milk Road - Defeat the Pea Hat (Day 1 only).
Hidden Hole outside Snowhead Area - Defeat the two Dodongos.
Outside the Great Bay fence - Defeat the two Bio Deku Babas and shoot down the beehive containing the Piece of Heart.
4 Gossip Stone Holes - Change the 4 Gossip Stones in each Hidden Hole to the same colour.
WOODFALL
Road to Southern Swamp - Clear your path of Bad Bats, then climb the vines to the top of the tree.
Tourist Information - Submit a picture of the Deku King in the Photography contest before clearing Woodfall Temple.
Swamp Shooting Gallery - Get a perfect score after you win the Bigger/Biggest Quiver.
Boat Cruise - Rescue Koume (the hag in the woods) and clear the Woodfall Temple. Then score 20 or higher in Koume's new Boat Cruise game.
SNOWHEAD
Goron Village - Trade the Swamp Title Deed to the Akindo Nut in the Goron Village area for the use of its Deku Flower; then launch youself to the Piece of Heart on the shelf above.
Twin Islands - Once you clear the Snowhead Temple, swim to this chest in the Twin Island area.
Snowhead - On the path to Snowhead Temple, after the 2nd set of ramps, look for a solitary platform. Use the Lens of Truth to reveal some invisible platforms, then summon the Scarecrow with the Scrawcrow's Song and Hookshoot over to the Piece of Heart.
Mountain Village - After clearing the Snowhead Temple, use the Don Gero's Mask to call together the 5 singing frogs (found in: Clock Town, Woodfall Temple, Southern Swamp, Great Bay Temple, and Mountain Village).
ROMANI RANCH
Doggie Racetrack - Bet on the fastest dog (using the Mask of Truth to figure out the dogs state of mind). When you win more than 150 Rupees, you get a Piece of Heart.
GREAT BAY
Marine Research Lab - Feed 4 bottles of fish to the fish kept in the aquarium by the door.
Zora Cape - Defeat the Like Like at the bottom of the waterfall's basin.
Zora Hall - Trade the Mountain Title Deed to the Akindo Nut in Lulu's room in return for use of its Deku Flower. Launch to the Piece of Heart on the ledge above.
Gerudo Fortress Sewers - In the under water passage into the Gerudo's Fortress, break the barrels covering the switch to the locked cages as Goron Link. Step on the switch, then dash (as Link) or roll into the cage to get the Piece of Heart.
Pinnacle Rock - Trade the shop keeper a picture of a Gerudo Pirate for the Golden Seahorse. Defeat all 8 Deep Pythons at Pinnicle Rock, then release the Seahorse when its mate appears.
Waterfall Rapids - After winning a bottle from the Beaver Bros, challange them a second time to win a Piece of Heart.
Cliffs by Gerudo Fortress Entrance - Use the Hookshot on the pillar closest to the wall seperating the Great Bay from the Gerudo's Lagoon. Plant a Magic Bean to travel to the next platform, then summon the Scarecrow with the Scarecrow's Song to hookshot over to the Piece of Heart.
Ocean Spider House - Use the Captains Hat to talk to the 6 Stalchildren, then shoot the coloured shields in the order they give you.
Jump Contest - After clearing the Great Bay Temple, travel by boat to the 5 islands in the northern part of the Great Bay (across from the entrance to Pinnacle Rock). Get 20 points or more to win the game and a Piece of Heart.
IKANA
Ikana Graveyard - Use the Captain's Hat on the second night to get the Stalchildren to move the gravestone. Inside, use the Lens of Truth to uncover a hidden passage. Defeat the Iron Knuckle to win a Piece of Heart.
Ikana Canyon - Use the Garo Mask to enter Ikana Canyon, then trade the Ocean Title Deed to the Akindo Nut across from Saken's Hideout for use of it's Deku Flower. Launch over to the Piece of Heart across the river.
Ghost Hut - Use the Garo Mask to enter Ikana Canyon and hookshot over to the village. Defeat the 4 Poe Sisters in the alloted time to win this Piece o' Heart.
Ancient Castle of Ikana - Use the Mirror Shield to enter the palace grounds. Take the door left of the entrance and climb to the top of the palace. Hit the crystal switch with an arrow, then use the Deku Flowers on top of the pillars to launch yourself to the Piece of Heart.
Secret Shrine - Use the Light Arrows to enter the Secret Shrine, then defeat the enemies inside to win the Piece of Heart.
MOON DUNGEONS
Deku Nut Dungeon - Use the four Remains Masks and the Oath to Order to enter the moon , then find the Piece of Heart in the mini dungeon.

in this I'll tell you what to do BENEATH THE GRAVEYARD.
There's one section of this per night.
On the first night, destroy all the Bad bats, light the torches, then do away with the Iron knuckle in the next room to learn the song of storms.
On night two, the lens of truth will be your best buddy, highlighting hidden doors, pathways, bad bats and skulltulas galore. The bombable wall near the end of it will lead to another Iron knuckle and a heart piece.
On night three, Z-target dampe and guide him to the earth mounds- get him upstairs by plonking him on a yellow block, then run up the blocks or ladder to reach the first floor yourself. Once dampe's unsettled three earth mounds, kill the poe that appears for an empty bottle.
